Scheduling
To make an appointment, please call the office at (954)399-2317 and leave a message or use the appointment form on the website. We will contact you within one to two business days.
Appointment Request
Follow-up Appointment Request:
Please call the office and leave your name, contact number(s), and several suggested preference days and times you would like to come in for an appointment. If you are in need of an urgent appointment please state this in your message.
New Evaluation Request:
Please leave your name and contact number(s), and suggested days and times you could come in for an appointment. Dr. Mendelsohn will try to call you within 2 business days to speak with you about making an appointment.
Fees
- Initial Evaluation (1-3 hours) | Starts at $570 for 1 hour
- 1 Hour | $570 1.5 Hours | $750 2 Hours | $1000
- Med Management/ Psychotherapy (30 minutes) | $285
- Med Management/ Psychotherapy (45 minutes) | $425
- Med Management/ Psychotherapy (60 minutes) | $500
- Additional Charges for Appointment Overrun | Prorated fraction of the $300 therapeutic hourly fee
- Form Completion/Letter Writing/Telephone Calls | Prorated fraction of the $300 therapeutic hourly fee
- Prescription written without an appointment | $20
- Prescription conveyed to pharmacy without appointment | $20
Fees are non-negotiable
Evaluations for new patients typically require several hours over one or more sessions.
Dr. Mendelsohn charge $300 for a 50 minute session and telephone consults are billed at the prorated session rate. Telephone fees do not apply to medication management issues. Routine medication issues or scheduling matters, such as making appointments, or raising questions regarding side effects do not typically incur fees, unless they are made emergently.
Insurance
Please note that Dr. Mendelsohn doesn’t participate with managed care or insurance companies, as they restrict my clinical care. If you decide to receive care from me, you can submit the receipts from your visits for reimbursement to your insurance company.
The receipts will have all the necessary information. Your insurance company will likely reimburse you a percentage of your payments based on an “Out of Network Provider” visit.
Some insurance companies will reimburse you at a higher rate or the full rate (as an ‘in Network Provider’) if you are unable to find someone on their panel who can see you in a reasonable amount of time.
